Job summary

ASHFORD CONNEX, LLC is seeking a Patient Care Manager (RN) to lead our home health team and elevate patient care in the Newport Beach area. This role requires clinical leadership, regulatory knowledge, and a commitment to compassionate service.

As a key clinical leader, you will guide staff, ensure adherence to standards and policies, and collaborate with families and referral sources to optimize patient outcomes and service delivery.

Qualifications

• Registered nurse with current licensure to practice in the state.
• Two years in a home care/supervisory role preferred.
• Knowledge of Medicare Conditions of Participation and regulatory requirements.

Qualifications:

• Previous experience in home care setting with two years management or supervisory experience, preferred.
• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
