Job summary

Avera McKennan Hospital is seeking a Registered Nurse to serve as the McKennan Patient Care Coordinator (PCC), coordinating complex care and connecting patients with needed services across hospital and home settings.

The PCC will help ensure smooth transitions, build relationships with patients, families, providers, and community partners, and focus on patient-centered care and outcomes.

Qualifications

• Active RN license in the state of practice.
• Ability to communicate effectively with patients, families and team members.
• 1–3 years home health/hospice experience preferred.

Responsibilities

• Coordinate patient's clinical services between hospital and home settings.
• Educate and coordinate services during care transitions, home health or hospice.
• Obtain physician orders and coordinate home care/hospice plans with the care team.
• Assist in assessing and identifying appropriate post-acute services in the home care setting.
• Maintain documentation of the home care plan and coordination activities.
• Facilitate communication between hospital/agency staff to improve continuity of care.
