Job summary

Avera Health is seeking an RN for home hospice nursing in the Sioux Falls area. The role involves providing in-home nursing care within a 40-mile radius of Sioux Falls and supporting patients and families with a collaborative multidisciplinary team.

The RN will perform assessments, develop and implement care plans, and evaluate patient progress while adhering to ANA standards. A sign-on bonus may be available and on-call duties are required roughly twice a month.

Qualifications

• Must hold an active RN license in the state of practice.
• Valid driver’s license and insurability required.
• Ability to communicate effectively with patients and team members.

Work where you matter.A Brief OverviewResponsible for utilizing the nursing process to provide nursing care for Avera@Home patients following the ANA Standards of Clinical Nursing Practice. The Nurse assumes responsibility for the outcome of nursing care provided to patients in their homes or in facilities. The Nurse demonstrates the knowledge and skills necessary to provide exceptional care for the population served.What you will doCollects patient health data.Analyzes the assessment data in determining nursing diagnoses/problem focus.Identifies expected outcomes/goals individualized to the patient.Develops a plan of care that prescribes interventions to attain expected outcomes.Implements the interventions identified in the plan of care.Evaluates the patient’s progress toward attainment of the outcomesSystemically evaluates the quality and effectiveness of nursing practice.Collaborates with the patient, significant others, and healthcare providers in providing patient care.Utilizes research/evidence-based findings in practice if appropriate.Considers factors related to safety, effectiveness, cost in planning, and delivering patient care.Essential QualificationsThe individual must be able to work the hours specified.
