Job summary

Veterans Health Administration is seeking an Registered Nurse for the Patient Aligned Care Team in an outpatient setting. The RN will provide direct veteran care, coordinate care across the team, and lead health promotion and disease prevention efforts.

The role emphasizes education, self-management support, and collaboration across interdisciplinary teams to optimize patient outcomes in a primary care environment. Schedule is Monday through Friday, 8am to 4:30pm, with no telework.

Qualifications

• The RN is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care.
• Assesses, plans, implements, and evaluates care based on age-specific components.

Responsibilities

• Provides direct Veteran care and care coordination within the Primary Care Department.
• Manages assigned panels, utilizes registries, ensures evaluation and access to patient panels.
• Provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management.
• Directs nursing education, orientation, competencies and quality improvement initiatives.
• Leads holistic primary care through collaborative strategies with others and evaluates practice against best evidence.
• Assists team with safe, quality outpatient nursing care and acts as a clinical resource for staff and patients.
