AMN Healthcare is seeking an RN Outpatient Care Coordinator in Sitka, AK for a 13-week assignment. You will coordinate patient care across providers, conduct assessments, and help ensure timely access to healthcare resources in a rural community.

Responsibilities include developing and monitoring care plans, facilitating referrals and follow-up appointments, and providing patient education while managing chronic conditions. Start date is ASAP with M-F days and Meditech experience preferred.

Formación

• RN license in Alaska (unrestricted).
• BLS certification.
• 1 year recent experience in a specialty.
• Knowledge of nursing principles and evidence-based practice.

POSITION SUMMARY – RN Outpatient Care Coordinator

Coordinates patient care across providers and services, conducts assessments, develops and monitors care plans, facilitates referrals and follow-up appointments, provides patient education, manages chronic and complex conditions, and helps ensure timely access to healthcare resources in a rural community.

Minimum Required Qualifications –

• Associate’s degree in Nursing from an accredited program.
• Current, unrestricted Registered Nurse license in the State of Alaska.
• Basic Life Support (BLS)
• 1 year recent experience in specialty
• Knowledge of nursing principles, evidence-based practice, medication safety, and infection prevention
