Medical Edge in Sitka, AK seeks an experienced RN Inpatient Care Coordinator to support patients during hospitalization by coordinating care plans and discharge planning. You will collaborate with physicians, nurses, social workers, and community resources to ensure continuity of care and a smooth transition home.

The ideal candidate has an RN license, BLS, and acute care or care coordination experience. Relocation and sign-on incentives are available along with comprehensive benefits.

Qualifications

• Associate Degree in Nursing; BSN preferred.
• Active and unrestricted RN license.
• Current BLS certification required.
• Strong clinical assessment and care coordination skills.
• Experience in acute care, case management, discharge planning or care coordination.
• Ability to work collaboratively within a multidisciplinary team.

Responsibilities

• Conduct comprehensive patient assessments and identify care coordination needs.
• Collaborate with physicians, nurses, therapists, social workers, and resources.

Pay Range: $41.66 - $58.69/hour

Position Summary We are seeking an experienced RN Inpatient Care Coordinator to support patients throughout their inpatient stay by coordinating care plans, facilitating discharge planning, collaborating with interdisciplinary teams, and ensuring continuity of care. This role combines strong clinical nursing skills with care coordination and patient advocacy to achieve optimal outcomes and a positive patient experience.

Responsibilities

• Conduct comprehensive patient assessments and identify care coordination needs
