Job summary

Trinity Health IHA Medical Group is seeking a compassionate Registered Nurse (RN) to join our outpatient care team in Michigan. You will perform clinical triage, care coordination, patient education, and post-discharge follow-up to improve outcomes and the patient experience.

You will participate in high-level nursing care, direct patient care, and care coordination for complex cases, while mentoring staff and collaborating with a broad care team.

Qualifications

• Graduate of an accredited Registered Nurse program.
• Valid, unrestricted RN license in Michigan.
• Valid CPR certification.
• Minimum 2 years of clinical experience in an ambulatory practice as an RN or demonstrated competency in IHA's RN position.

Responsibilities

• Perform high-level nursing care, wellness care, and chronic disease education/management.
• Provide direct care to individual patients and/or populations.
• Coordinate care for patients with complex needs.
• Assess and triage patient needs by phone and in-office visits.
• Provide clinical guidance and patient education.
• Support transitions of care and post-discharge follow-up.
• Collaborate with providers, care managers, pharmacists, BH specialists, and other healthcare partners.
• Manage medication refills, test results, referrals, and prior authorizations.
• Identify preventive care needs and support chronic disease management.
• Help patients access community resources and navigate barriers to care.
• Serve as a clinical resource and leader within the care team.
