Job summary

Avera Comprehensive Breast Care in Sioux Falls seeks an experienced Registered Nurse to help coordinate cancer care across the continuum in collaboration with multiple departments. The role focuses on removing barriers to care and ensuring optimal outcomes for patients through effective team collaboration.

You will document assessments and referrals in the EMR, participate in multidisciplinary reviews, and educate patients and families while aligning with clinical guidelines and care plans.

Qualifications

• Registered Nurse (RN) license in the state of practice
• Ability to coordinate care across multiple departments
• Strong communication and documentation skills in EMR

Responsibilities

• Facilitate coordination of patient care services and ensure timely appointments and referrals
• Document patient assessments, education, referrals, and activity in the EMR
• Contribute to development and adherence of clinical guidelines and care plans
• Support multidisciplinary care conferences and case reviews for coordinated treatment
• Educate patients and families; develop educational materials and resources
• Collaborate with physicians and teams to coordinate care across locations and specialties
• Assist administration with goals, data collection, and impact evaluation of navigation processes
• Use triage protocols to place patients with appropriate specialties and care locations
