Job summary

Barnes-Jewish Hospital is seeking a registered nurse to coordinate care across the continuum for designated patient populations. Responsibilities include assessing patient needs, coordinating safe care, and educating patients and families.

The role requires RN licensure and 2–5 years of nursing experience; a BSN is preferred. This position is remote with residency in the Greater St. Louis area and a daytime schedule, Monday through Friday, 8:00 am to 4:30 pm.

Qualifications

• Education: Nursing Diploma or Associate's in Nursing.
• Experience: 2–5 years in nursing.
• Licenses: RN license.
• Preferred: Bachelor's degree in Nursing.

Responsibilities

• Coordinates care for designated patient populations across the continuum.
• Assesses patient physical, psychosocial, and emotional needs and educates patients/families.
• Facilitates safe and evidence-based patient care and monitors outcomes.
• Acts as a resource to the healthcare team and supports referrals as needed.
