Job summary

Central Minnesota Mental Health Center (CMMHC) is seeking a Registered Nurse to join the Assertive Community Treatment (ACT) Team in St. Cloud, MN.

The RN will manage the medication system, administer and document treatments, and collaborate with the Psychiatrist, NP or CNS to provide comprehensive care to individuals with severe mental illness. The role requires Minnesota RN licensure, CPR/First Aid, and a valid driver’s license.

Qualifications

• Licensed RN in Minnesota required.
• RN experience on ACT teams preferred.
• Valid Driver's License with clean driving record.
• CPR and First Aid certification required.

Responsibilities

• Develop, revise, and ensure adherence to medication policies and procedures, including transcription, administration, and documentation.
• Administer and document medications; coordinate with psychiatrists, NPs or CNS.
• Evaluate medication effectiveness, side effects, and patient reactions; document findings.
• Provide medication education to recipients and assess readiness to learn.
• Identify training needs for ACT team members on psychotropic medications.
