Job summary

Central Minnesota Mental Health Center seeks a Mental Health Professional or Clinical Trainee to serve as ACT Team Lead for the St. Cloud ACT team.

This role provides clinical leadership to a multidisciplinary team delivering community-based, person-centered support, guiding crises response, and coordinating services to help clients build stability. The position offers a Monday–Friday schedule 7:30 am–3:30 pm with rotating after-hours coverage.

Qualifications

• Licensed as LP, LPCC, LICSW, or LMFT, or a Clinical Trainee actively working toward licensure.
• Clear understanding of the Assertive Community Treatment service delivery model.
• Ability to provide clinical leadership and make sound decisions in complex situations.
• Ability to work effectively as part of a multidisciplinary team.
• Successful completion of a DHS background study.
• Valid driver's license and acceptable driving record.

Responsibilities

• Lead as a team with shared responsibility, strong communication, and collaborative decision‑making across the multidisciplinary ACT team.
• Meet people where they are; support community-based services in clients' homes, neighborhoods, and other community settings.
• Respond to complex needs; provide clinical leadership as the team handles psychiatric symptoms, crises, housing instability, and related challenges.
• Keep recovery at the center; help clients build stability, independence, meaningful relationships, employment, and housing.
• Coordinate the whole picture; assess, refer, coordinate, and monitor services while reducing gaps in care.
• Bring resources to the table; connect clients with mental health, medical, housing, and social services.
• Provide after-hours clinical leadership; oversee ACT team during off-business hours as needed.
• Build a strong team; oversee staffing, training, coaching, and performance evaluations.
• Lead program operations; support financial viability and secure necessary resources.
