Job summary

Hartford HealthCare Medical Group is seeking a Registered Nurse to provide skilled nursing care in a variety of clinical settings under provider direction. The role involves office-based nursing, patient assessment, and care planning to ensure continuity of care.

You will triage calls, coordinate with physicians, and support patients and families while maintaining confidentiality and quality standards. Travel between Hartford offices is required.

Qualifications

• Current Connecticut RN License required.
• Previous experience in Medical/Surgical Office setting preferred.
• Experience with medical software

Responsibilities

• Provides office-based nursing care in collaboration with provider.
• Assesses physical and psychosocial needs and environmental factors affecting patients and their families, formulates nursing plan of care and ensures continuity of patient care.
• Demonstrates independent nursing actions based on assessment and problem identification.
• Triages telephone calls and initiates appropriate nursing intervention in prompt, precise, and professional manner
