Job summary

Hartford HealthCare Medical Group in Hartford, CT is seeking a skilled Registered Nurse to join our clinical team. Under provider direction, you will deliver office-based nursing care, assess patient needs, and participate in care planning and continuity.

You will triage calls, support families, travel to regional offices as needed, and collaborate with providers to ensure high-quality patient care. CT RN license and CPR are required; three years in a medical practice preferred.

Qualifications

• Current state of Connecticut RN license and CPR certification required.
• Three years of experience in a medical practice preferred.
• Experience with medical software and EMR systems beneficial.

Responsibilities

• Triage telephone calls and provide appropriate nursing intervention.
• Provide office-based nursing care in collaboration with provider.
• Assess physical and psychosocial needs and plan care; ensure continuity of patient care.
• Demonstrate independent nursing actions based on assessment and problem identification.
• Travel to other Hartford HealthCare regional offices as needed.
