Job summary

Hartford HealthCare Medical Group in Connecticut is seeking a Registered Nurse to provide skilled nursing care across clinical settings under provider direction. You will use the nursing process to assess, plan, intervene and evaluate patient needs, reporting to a Practice Manager.

Responsibilities include collaborative office-based care with providers, triaging calls, and supporting families with empathy while maintaining confidentiality. Travel to regional offices may be required.

Qualifications

• Current state of Connecticut RN license and CPR certification.
• Three years of experience in a medical practice (preferred).
• Experience with medical software/EMR systems.

Responsibilities

• Provide office-based nursing care in collaboration with provider.
• Assess physical and psychosocial needs; formulate care plans and ensure continuity of care.
• Demonstrate independent nursing actions based on assessment and problem identification.
• Triage telephone calls and initiate appropriate nursing interventions promptly.
