Hartford HealthCare in Wilton, CT is seeking an experienced registered nurse for office‑based care. Under provider direction, you will assess, plan, intervene and evaluate patient needs using established nursing processes, collaborating with physicians and staff to ensure continuity of care.

You will triage calls, support families with empathy, travel to regional offices as needed, and maintain confidentiality.

Formación

• Current state of Connecticut RN license and CPR certification.
• Three years of experience in a medical practice.
• Previous experience in a medical/surgical office setting preferred.
• Experience with medical software.

Responsabilidades

• Provides office-based nursing care in collaboration with the provider.
• Assesses physical and psychosocial needs, formulates plan of care, and ensures continuity of patient care.
• Demonstrates independent nursing actions based on assessment.
• Triage telephone calls and initiate appropriate nursing interventions promptly.
• Assists patients and families with empathy while maintaining confidentiality and privacy.
• Travels to other regional offices as needed.
