This role provides visible clinical leadership within a post-acute, subacute rehabilitation setting, with collaboration from DON, ADON, and regional support to maintain quality and regulatory readiness.

Qualifications

• Requires RN license and current registration to practice as a Registered Nurse
• Minimum of two years of nursing experience and at least one year in a leadership role
• Interest or experience in geriatrics and dementia care preferred
• Current BLS/CPR certification
• Ability to read, write, and understand English at an intermediate level

Responsibilities

• Supervise nursing care services and personnel across assigned units
• Conduct rounds and provide clinical support and guidance to staff
• Monitor resident conditions and communicate changes promptly
• Assist with admissions, evaluations, care planning, and resident oversight
• Support safe medication administration and treatment delivery
• Coordinate staffing and provide coaching to nursing staff
• Ensure documentation and workflows meet policy standards
• Participate in shift-to-shift communications and updates
• Foster a collaborative, respectful team environment
• Maintain CMS standards and regulatory readiness
