Job summary

UNC Health Care in Chapel Hill, NC is seeking a Clinical Nurse Coordinator to provide clinical administrative nursing care coordination in support of a patient care area. Duties include care coordination, discharge planning, data analysis and performance metrics, and patient/student education.

The position emphasizes collaboration with an interdisciplinary team, patient and family communication, and contributions to performance improvement and staff education.

Qualifications

• Graduation from a state-accredited school of professional nursing.

Responsibilities

• Assesses patient status of a specialized population and coordinates care with the interdisciplinary health care team.
• Coordinates post-hospitalization care and discharge planning, including appointments with clinics, home care, and durable medical equipment providers.
• Provides inservices and presents to nursing, medical and ancillary staff on topics related to the specialized patient population; develops patient education materials.
• Collects data related to the specialized patient population, analyzes trends, and participates in performance improvement activities to promote quality care.
• Promotes ongoing professional development and models nursing practice through the use of the nursing process.
