Job summary

Live Well Homecare is seeking an experienced RN Nurse Navigator Triage to provide nursing triage services, assess patient acuity, offer health advice, and coordinate care over the phone and in person. In this full-time clinical role, you will perform patient assessments, document symptoms, initiate emergency care when needed, educate patients, schedule appointments, and liaise with multiple departments.

A BSN is preferred with 5+ years in ICU/ED or critical care, and proficiency with computer

• Proficiency in computer systems and Microsoft Office is required.
• Strong communication, clinical judgment, and coordination abilities are essential.

Responsibilities
