Qualifications

• Graduate from Specialty Training Program- (Required).
• Bachelor's Degree-Nursing (Preferred).
• Minimum of 2 years-Related work experience (Required).
• Licensed Registered Nurse (Pennsylvania) and BLS.

Responsibilities

• Coordinate pre and post hospital care for patients.
• Serve as liaison between patient and care team.
• Coordinate multidisciplinary clinics and patient navigation.
• Provide patient education and ongoing continuity of care.
• Assist provider and patients with diagnosis, treatment, and follow-up.
