Qualifications

Responsibilities

• Visit geriatric clients in Bethlehem, PA and surrounding areas for nursing assessments.
• Develop, manage, and oversee client care plans and coordinate communication among clients, families, caregivers, clinicians and staff.
• Provide clinical leadership and supervision to Home Health Aides and field staff to ensure quality of care.
• Mentor and resource for caregivers to support professional growth and service delivery.
• Facilitate orientation and onboarding for new Home Health Aides and CNAs.
• Participate in training, coaching, and competency development for field staff.
• Collaborate with office leadership on staffing, client satisfaction, engagement and quality improvement.
• Maintain compliance with policies, state regulations and industry best practices.

Qualifications for a Clinical Nurse Manager:

• Active Registered Nurse (RN) license required.
• Minimum of 2 years of experience as a Registered Nurse required.
• Home Care, Home Health, Hospice, Private Duty, Case Management, or Community Health experience preferred.
• Supervisory, Charge Nurse, Clinical Lead, Preceptor, or Nurse Manager experience preferred.
• Strong assessment, care planning, and clinical decision-making skills.
• Excellent communication, organizational, and leadership abilities.
