ROLE RESPONSIBILITIES - as RN, Nurse Home Care Case Manager, you oversee the nursing standards for the Caring for Family of Companies. You're responsible for the highest quality of case management and care for Clients - as well as providing high caliber Caregiver training and professional development. You maintain strict compliance with established protocols and the In-Home Care licensing standards. This position reports to the Director of Nursing Services, and collaborates with an entire team of Nurses for ongoing support.

  • Oversee nursing services, including medication administration program
  • Ensure standardization and adherence of process
  • Maintain Client confidentiality
  • Review medication administration and medication error reports to maintain safety and compliance protocols
  • Review medication orders to confirm Client needs and contradictions, collaborating with providers for adjustments when needed
  • Conduct nursing and medication evaluation and assessment visits, addressing Client concerns
  • Train Caregivers on medication administration and evaluate competency
  • Train Caregivers on Nurse-delegated duties, ensuring competency and ability to deliver care
  • Travel to regional branches to fill staffing gaps, perform nursing assessments, and train on medication administration - as needed
