Offers primary clinical assistance to providers, patients, and families for diagnosis and treatment, including triaging patient communications and creating individualized care plans.

Key Responsibilities

• Assess healthcare, educational, and psychosocial needs, involving patients and families in treatment planning.
• Facilitate communication among healthcare team members and explore healthcare options to eliminate barriers.
• Educate patients and families on disease management, care coordination, and program outcomes using age-appropriate techniques.
• Implement clinical interventions based on evidence-based guidelines and risk stratification.
• Collaborate with primary care providers to ensure guideline adherence, initiate therapies, and provide follow-up.
• Coordinate diagnostic and laboratory testing, document activities in electronic health records, and collect patient data.
• Participate in ongoing professional development and incorporate current research into practice.
