Azza HealthCare Agency is seeking an experienced Registered Nurse to provide advanced clinical support within an interdisciplinary team in the Wilkes-Barre area. You will assess patient needs, coordinate care, and educate families while following clinical guidelines and participating in ongoing education.

Responsibilities include coordinating tests, managing care plans, administering medications, and supporting transitions of care in a primarily clinical setting.

Qualifications

• Minimum of 2 years of RN work experience.
• Graduate from Specialty Training Program in Nursing (required); Bachelor's Degree in Nursing (preferred).

Responsibilities

• Assess healthcare, educational, and psychosocial needs, involving patients and families in treatment planning.
• Facilitate communication among healthcare team members and investigate healthcare options to eliminate barriers.
• Educate patients and families on disease management, case management, and care coordination using age-appropriate methods.
• Implement clinical interventions based on risk and evidence-based guidelines, collaborating with providers to initiate and adjust therapies.
• Coordinate laboratory and diagnostic tests, document activities in electronic health records, and collect patient data.
• Participate in continuing education and incorporate current literature into practice.
• Support post-discharge transitions of care and assist in designing, implementing, and evaluating care programs.
• Prepare and handle laboratory specimens, administer medications and vaccines, and manage IV therapy within scope of practice.
