Job summary

Cardinal McCloskey Community Services (CMCS) is seeking a Registered Nurse - Nurse Care Manager to oversee medical care for individuals in our group homes and day habilitation programs. The RN provides medical consulting, triage, and education to staff, ensuring safe medication practices and up-to-date nursing services.

Qualifications

• Registered Nurse with current New York State Licensure.
• Experience in health care management for individuals with developmental or psychiatric disabilities preferred.
• Must have daily access to an automobile and be willing to travel between locations in Bronx and Westchester.

Responsibilities

• Provide care management, medical consultation and triage for individuals with disabilities.
• Develop and review Plans of Nursing Services (PONS) annually and as needed; train staff accordingly.
• Monitor medication administration, MARs, and ensure compliance with OPWDD standards; address errors and disposals.
• Conduct weekly site visits to review medical records and medication status; educate teams on medical needs.
