Job summary

Lexington Premier Nursing & Rehab is seeking a compassionate Registered Nurse (RN) to join our skilled nursing facility team. You will provide high-quality patient care, administer medications, and coordinate with other healthcare professionals to ensure the best outcomes for our residents.

The role requires an ADN or BSN, a Kentucky RN license, and BLS certification; ACLS is preferred. Expect to mentor nursing staff and participate in rotating weekend, night, and holiday calls while maintaining

• 1 year of nursing experience in a skilled nursing or long-term care setting preferred.

Responsibilities

• Provide direct patient care including assessments, treatment planning, and administering medications.

• Communicate with residents, families, and healthcare team members.
• Maintain accurate patient records and documentation in line with policies and standards.

• Respond promptly to medical emergencies and provide stabilization.
• Participate in weekend, night, and holiday call rotation.

Job Summary:

Lexington Premier Nursing & Rehab is seeking a compassionate and dedicated Registered Nurse (RN) to join our skilled nursing facility team. As a Registered Nurse, you will be responsible for providing high-quality patient care, administering medications, and coordinating with other healthcare professionals to ensure the best outcomes for our residents.

Responsibilities:

• Patient Care: Provide direct patient care including assessments, treatment planning, and administering medications. Monitor and document patient progress and respond to changes in condition.
• Coordination of Care: Collaborate with physicians, therapists, and other healthcare professionals to develop and implement individualized care plans for residents.
• Communication: Effectively communicate with residents, families, and healthcare team members to address concerns and provide updates on patient status.
• Documentation: Maintain accurate and up-to-date patient records, including assessments, interventions, and outcomes, in compliance with facility policies and regulatory standards.
• Training and Supervision: Mentor and guide nursing staff, including LPNs and CNAs, ensuring adherence to care protocols and facility procedures.
• Compliance: Ensure all nursing practices comply with state and federal regulations, including infection control, safety protocols, and resident rights.
• Emergency Response: Respond promptly and appropriately to medical emergencies, utilizing critical thinking and clinical judgment to stabilize and manage acute situations.
• Participation: Works with other nurses in taking weekend, night, and holiday calls on a rotating basis.

Qualifications:

• Education: Associate’s Degree in Nursing (ADN) or Bachelor’s Degree in Nursing (BSN) from an accredited program.
• Licensure: Current and valid Registered Nurse (RN) license in Kentucky.
• Experience: 1 year of experience in a skilled nursing or long-term care setting preferred, but not required.
• Certifications: Basic Life Support (BLS) certification required; Advanced Cardiovascular Life Support (ACLS) certification preferred.

• Proficient in electronic health records (EHR) and basic computer applications.
• Excellent communication and interpersonal skills.
