Job summary

Inland Empire Health Plan (IEHP) is seeking a Non-Routine Placement Nurse, RN to lead complex patient placement and transition coordination across facilities and care levels in a hybrid work setting. You will collaborate with hospitals, IPAs, SNFs, CLHFs, and post-acute partners to ensure timely, cost-effective care transitions.

Qualifications

• 2+ years in a health care delivery setting in Acute Care Hospital or Skilled Nursing Facility.
• Experience in a health plan setting preferred.
• Active, unrestricted RN license issued by the California BRN.

Responsibilities

• Lead coordination of complex or non-routine patient placements and discharges across facilities and care levels.
• Act as primary contact for escalated cases; ensure timely follow-up and communication of status.
• Facilitate transitions to higher or lower levels of care and support discharge planning.
• Provide education and guidance to case managers on complex placements and coordination.
• Maintain placement logs and EMR documentation; support audits and QA efforts.
• Ensure regulatory compliance with CMS, HEDIS, CAHPS, and related standards.
• Collaborate with providers, hospitals, IPAs, and post-acute partners to optimize care pathways.
