Job summary

Inland Empire Health Plan seeks a Non-Routine Placement Nurse, RN, to lead complex patient placements across care settings. You will coordinate with hospitals, IPAs, SNFs, CLHFs and other post-acute providers to ensure timely, high-quality transitions while maintaining regulatory compliance.

This hybrid role requires California RN license, 2+ years in healthcare delivery, strong analytical and communication skills, and the ability to work with cross-functional teams to optimize patient outcomes

Qualifications

• Two (2) or more years in a health care delivery setting in Acute Care Hospital or Skilled Nursing Facility.
• Active, unrestricted Registered Nurse (RN) license issued by the California BRN.
• Experience in a health plan setting preferred.

Responsibilities

• Lead coordination of complex or non-routine patient placements and discharges across facilities and levels of care.
• Review clinical information to determine appropriate placement based on acuity and service needs.
• Partner with case managers, social workers, hospitals, IPAs and post-acute partners to resolve barriers.
• Monitor patient movement and throughput, escalate delays or safety concerns.
• Provide education and guidance to case managers and team members on processes for complex placements and discharge coordination.
• Maintain placement logs and ensure accurate documentation for audits and reporting.
