Job summary

Inland Empire Health Plans (IEHP) is seeking a Non-Routine Placement Nurse, RN to lead complex patient placement coordination across the continuum of care in California. You will collaborate with hospitals, IPAs, SNFs, CLHFs and post-acute providers to ensure timely, cost-effective transitions for high-risk members.

The role functions as a clinical and operational liaison, guiding level-of-care determinations, removing placement barriers, and supporting discharge planning and readmission

Qualifications

• Two or more years in a health care delivery setting in Acute Care Hospital or Skilled Nursing Facility.
• Active, unrestricted RN license issued by the California BRN is required.
• Experience in a health plan setting is preferred.

Responsibilities

• Lead coordination of complex and non-routine patient placements and transitions.
• Partner with hospitals, IPAs, SNFs, CLHFs and post-acute providers to ensure timely, cost-effective care transitions.
• Serve as clinical and operational liaison for level-of-care determinations and barrier removal.
• Support discharge planning, transitional care, and readmission prevention with regulatory compliance.
