Job description

RN Night Shift – 36 Hours – 7pm–7am

Location: Simsbury, Connecticut, United States

Position Summary: Provides nursing care to residents using the principles of the nursing process from assessment through evaluation. Follows the CT Nurse Practice Act and McLean Nursing Philosophy of Excellence, Leadership, Integration, Learning and Discovery, while administering and directing ancillary personnel in the performance of nursing tasks. Takes responsibility for the delivery of care on the unit and engages in proper hand‑off to the next shift.

Key Responsibilities:

  • Arrive for duty on time and ready to assume responsibility for the unit when assigned
  • Organize resident/patient care according to accepted routines and prioritize activities, managing work so as to complete duties within the eight‑hour shift
  • Conduct ongoing assessments of residents’ nursing care needs and immediately report changes in condition with accurate assessments and interventions to supervision, APRN or physician
  • Transcribe orders from physicians in a timely manner following consult or post‑physician visit
  • Assist the MDS Coordinator in developing the plan of care for each resident/patient
  • Administer medications and patient treatments as prescribed, documenting care and resident response
  • Assist in patient and family teaching and education, documenting patient understanding in the medical record for both formal and informal sessions (e.g., medications, activities of daily living, treatments, procedures)
  • Take and assess vital signs as indicated by the plan of care or changes in patient condition
  • Wearing the unit pager at all times and responding when signaled, except during lunch period
  • Read and follow PCC messaging
  • Carry out approved emergency procedures
  • Prepare and follow up on “As & Is” documentation to completion
  • Monitor IV therapy, document, and discontinue as needed
  • Follow Standard Precautions and ensure procedures are followed by ancillary staff
  • Participate in and/or provide information for resident planning conferences
  • Communicate resident information to other members of the healthcare team and to residents’ families
  • Maintain a safe environment for residents, staff, and visitors and report any concerns to supervisor
  • Assign resident assignments for ancillary staff, review and monitor patient care performed by ancillary personnel, collect and document quality care indicators, and provide feedback to supervision
  • Support department and unit initiatives, promptly notifying supervisor or nurse educator of learning needs for nursing care
  • Manage time appropriately to complete duties within the eight‑hour shift, requesting supervisory assistance when necessary to ensure compliance before shift end
  • Perform other activities as assigned

Employment Requirements: Knowledge of nursing theory and practice, competency in the nursing process, quality assurance, infection control, hospice philosophy, restorative, rehabilitative, and gerontological nursing practice and theory. Ability to interpret written instructions, communicate and document effectively, prioritize appropriately, and make sound nursing judgments. Demonstrates personal integrity and empathy and fosters such in others. Graduate of an accredited Registered Nurse program. Current Connecticut State License in good standing as a Registered Nurse. Minimum of one year of experience in long‑term care working with a geriatric population.
