Job summary

Mayo Clinic in Phoenix, AZ is seeking a dedicated registered nurse (RN) to coordinate patient care across the continuum. The role includes direct patient care, patient and family education, and active participation in transitions of care within Mayo Clinic's Nursing Professional Practice Model.

The RN will provide leadership through preceptor responsibilities and teamwork, ensure high standards of care, and adapt to flexible hours.

Qualifications

• Graduate of a baccalaureate nursing program as recognized by ACEN/CCNE/NLN CNEA.
• At least one year RN experience in an applicable care setting or one year of LPN experience at Mayo Clinic is preferred.

Responsibilities

• Coordinate nursing care across direct patient care, patient/family education and transitions of care.
• Provide leadership through mentoring, preceptor roles and quality improvement activities.
• Demonstrate effective communication and collaboration with the care team.
