Job summary

Southern Illinois Healthcare seeks an RN navigator to coordinate care for neurology or neurosurgery patients in Carbondale, IL. You will review diagnoses, coordinate pre-appointment care, and guide referrals to designated facilities.

You will work with physicians and other professionals to ensure timely, integrated care across the care continuum. Responsibilities include triage, documentation of navigation activities, and collaborative program development.

Qualifications

• Three years of technical nursing experience
• Current Illinois RN licensure
• Associate/Bachelor/Master in Nursing preferred

Responsibilities

• Serve as a clinical expert to help neurosurgery or neurology patients and families navigate the health care system.
• Provide initial coordination of care for patient receiving pre-appointment treatment.
• Review patient’s diagnosis, medical history, and physician disposition and communicate information to patient.
• Direct patients to facilities listed on the Network Provider list when pre-appointment treatment is recommended and facilitate referrals to these facilities
• Assist physicians in coordination of care of neuroscience or neurology patients with other health care professionals.
• Refer patients to other community resources, as necessary
• Provide ongoing navigation for patients receiving pre-appointment treatment
• Communicate written treatment completion information to the Primary Care or referring physician if pre-treatment results in symptom resolution and patient does not require an appointment with the Southern Illinois Healthcare Brain and Spine Institute
• Provide ongoing documentation of nurse navigation activities on each patient, including the time involved in these activities
• Assist in the development, documentation and analysis of program performance
• Serve as a clinical resource to Intake Specialists
• Collaborate with other department members to facilitate effective triage of new neuroscience or neurology patients
• Eliminate unnecessary delays in clinician reviews, including supervising the collection of patients’ diagnostic studies
• Return Brain and Spine Institute messages within 24 hours
• Triaging neurology referrals and managing transitional care for discharged neurology patients including imaging, wellness checks, referrals, appointments and medications
• Follow up on Emmi report indications as needed

• Responsible for all triaging of neurology referrals sent to Brain and Spine
• Responsible for transitional care management of discharged neurology patients including: imaging, wellness checks, referrals, appointments and medication refills/order
• Responsible for any follow up indicated on Emmi report
