Conducts thorough assessments, develops and evaluates individualized care plans incorporating evidence-based practices, and documents interventions and patient responses. Provides education, discharge planning, and coordinates referrals. Recognizes and responds to changes in patient condition, administers medications, and ensures safety and infection control standards are met.
Valid RN license in applicable states, BLS certification, strong communication, critical thinking, and collaboration skills. Ability to adapt to changing demands, maintain professionalism, and promote a respectful environment.
Participates in quality improvement, performance initiatives, and evidence-based practice activities. Demonstrates age-appropriate care for adult and geriatric patients and adheres to safety protocols. Provides service excellence by respecting patient dignity, ensuring confidentiality, and fostering teamwork.
