Qualifications

• RN—Bachelor’s degree in Nursing; CA RN license.
• Two years clinical experience.

Responsibilities

• Initiates and coordinates patient/family teaching regarding care and follow-up.
• Reviews medications and documents history; evaluates patients.
• Coordinates consults with physicians and services.
• Prepares patient for visit; gathers reports for MD review.
• Provides triage for patient care concerns and acts as liaison with the health-care team.
• Oversees patient flow and scheduling of tests, procedures, and admissions.
