Job summary

MercyOne Iowa Heart Center Des Moines is seeking an RN Navigator: Heart Failure to lead care coordination and education within our Center of Excellence for Heart Failure Management.

You will guide patients and families through transitions, collaborate with a multidisciplinary team, and support discharge planning and post-discharge follow-up. Schedule: Full time, 40 hours/week, 8:00-4:30, No Weekends or Holidays; Iowa RN license required.

Qualifications

• Registered nurse with valid Iowa license is required.
• BSN degree preferred.
• Minimum 2 years of related experience; 4 years preferred.
• Excellent communication and presentation skills; ability to educate patients and families.

Responsibilities

• Develop and implement a Heart Failure Center of Excellence for management guidelines.
• Navigate care transitions and coordinate with multidisciplinary team.
• Provide patient and family education and advocacy.
• Coordinate discharge planning and post-discharge follow-up.
• Collaborate with physicians, nurses, and extended care providers to ensure continuity of care.
