Job summary

MercyOne Health System in Des Moines, IA invites you to join as an RN Navigator: Heart Failure at MercyOne Iowa Heart Center Des Moines. This full-time role focuses on coordinating care, patient education, and transitions across the cardiovascular pathway, from hospital to home, with no weekends or holidays.

You will work under the Cardiovascular Service Line to support patients and families with compassionate, evidence-based care.

Qualifications

• RN nursing, care coordination, patient education, teamwork
• Excellent communication and advocacy
• Clinical excellence in cardiovascular care
• Collaborative with multi-disciplinary teams

Responsibilities

• Coordinate care across diagnosis to discharge for heart failure patients
• Perform daily rounds on heart failure patients
• Provide patient/family education and discharge planning
• Collaborate with physicians, nursing staff and extended care providers
• Support post-discharge follow-up and care transitions
• Participate in data collection and quality initiatives
