Job summary

MercyOne Heart Center Des Moines seeks a RN Navigator: Heart Failure to develop and implement a heart failure management program based on evidence-based guidelines. You will promote care transitions, educate patients and families, and coordinate across hospital and outpatient services within a collaborative cardiovascular team.

The role reports to the Director of Cardiac Specialty Services and requires strong communication, advocacy, and care coordination across the continuum.

Qualifications

• Registered nurse with valid license in Iowa required.
• Bachelor's degree in healthcare field or actively pursuing, BSN preferred.
• Two years of related work experience required, 4 years preferred.
• Proof of completion of Mandatory Reporter abuse training within 3 months of hire.
• Excellent communication skills and presentation skills with groups.

Responsibilities

• Coordinate care for patients throughout the continuum from diagnosis to rehabilitation.
• Reconcile daily census to identify heart failure patients and perform daily rounds.
• Educate patients and families; coordinate discharge planning and follow-up.
• Collaborate with physicians, nursing staff, and extended care providers to ensure quality outcomes.
• Participate in data collection and quality improvement initiatives during hospitalization.
