Job summary

CommonSpirit Health is seeking a dedicated RN Navigator to coordinate care across the clinical team within a physician office setting. This role emphasizes safety, quality, and adherence to best practices while managing program oversight, registry maintenance, and care transitions.

You will perform Medicare AWVs, maintain disease registries, generate provider reports, and support grant-related initiatives. Strong teamwork and proactive patient care are essential to success in this rural health

Qualifications

• Associates degree in nursing required.
• Minnesota RN license required.
• Bachelor's degree in nursing preferred.

Responsibilities

• Coordinate RPM program using real-time data to improve outcomes.
• Maintain the disease registry and identify overdue patients.
• Generate performance reports for providers and clinic leadership.
• Support grant-related initiatives and track key metrics for success.
• Coordinate and conduct Medicare Annual Wellness Visits (AWVs) according to CMS guidelines.
• Perform pre-visit chart reviews to identify gaps in preventive care and trigger standing orders.
