This full-time, day-shift role links nurses, behavioral health clinicians, psychiatrists, patients, and community resources to ensure integrated care for patients with depression, anxiety, and related conditions.

Qualifications

• Bachelor's degree in nursing or related field required.
• Current RN license in the state of practice required; BLS certification required.
• Proficiency with clinical documentation and health records; strong organizational skills.
• Minimum 5 years of clinical experience; knowledge of care coordination and management.

Responsibilities

• Assess patient plan of care and coordinate resources to meet health care needs.
• Evaluate medical necessity and care appropriateness; liaise with physicians and schedule tests/consults.
• Coordinate disease care with an interdisciplinary team; refer to specialists as needed.
• Explain diagnostic processes and care plans to patients and families; provide information and resources.
• Set functional, measurable treatment goals; initiate discharge planning with patient/family.
