Job summary

Tallahassee Memorial Healthcare (TMH) in Tallahassee, FL, seeks a Nurse Navigator to support patient journeys from diagnosis to survivorship. You will coordinate referrals, enable timely transitions of care, and partner with multidisciplinary teams to optimize outcomes for our diverse patient population.

Requirements: Florida RN license and at least two years of clinical care or navigation experience; BSN preferred. Oncology specialty certification is a plus.

Qualifications

• Associate degree in nursing is required.
• Florida RN license is required.
• Two years of clinical care, case management, or navigation experience.
• Bachelor's degree in nursing is preferred.
• Speciality Certification (e.g., Oncology) is preferred.

• Ensures timely and efficient patient referrals and transition throughout the continuum of care from diagnosis to survivorship.
• Responsible for the management of resources and services through interdisciplinary collaboration to achieve optimal, contiguous patient outcomes.

Qualifications

Required Education:

• Associate degree in nursing

Preferred Education:

Required Experience:

• Two (2) years of clinical care, case management, or navigation experience

• Five (5) years of related specialty care

Required Certification/License/Registry:

• Florida registered nurse (RN) license

Preferred Certification/License/Registry:

• Speciality Certification (i.e. Oncology)
