Job summary

Tallahassee Memorial Healthcare (TMH) is seeking a Nurse Navigator to support timely referral and transition along the care continuum from diagnosis to survivorship, collaborating across disciplines to optimize patient outcomes.

The role reports to RN/Nurse Manager and focuses on coordinating resources and services to ensure seamless care delivery across departments and programs within our 22-county service area.

Qualifications

• Required Education: Associate degree in nursing
• Preferred Education: Bachelor's degree in nursing
• Required Experience: Two (2) years of clinical care, case management, or navigation experience
• Preferred Experience: Five (5) years of related specialty care
• Required Certification/License/Registry: Florida registered nurse (RN) license
• Preferred Certification/License/Registry: Specialty Certification (i.e. Oncology)

Responsibilities

• Ensures timely and efficient patient referrals and transition throughout the continuum of care from diagnosis to treatment to survivorship.
• Responsible for the management of resources and services through interdisciplinary collaboration to achieve optimal, contiguous patient outcomes for patients.
