Job summary

CommuniCare Health Services is seeking a Registered Nurse Mobile MDS Coordinator to support our Midwest division across skilled nursing facilities. You must hold an Ohio RN license and bring MDS/RAI experience, with senior or multi-facility MDS background preferred.

The role requires travel and in-facility collaboration with residents, families, and staff. We offer a competitive salary with PTO, comprehensive benefits, a 401(k) with match, and Flexible Spending Accounts.

Qualifications

• Current RN license in Ohio, active and in good standing.
• At least 2 years’ experience with Federal Medicare/Medicaid and MDS/RAI.
• Senior MDS or multi-facility MDS experience preferred.
• AANAC certification highly preferred.
• Excellent written and verbal communication skills.
• Willingness to travel across Ohio and occasionally to Indiana.

Responsibilities

• Evaluate facility compliance to federal regulations and utilization targets.
• Weekly facility visits for intervention, audits and training.
• Review resident records for coverage, utilization and transmission accuracy.
• Provide consultation on federal/state reimbursement systems.
