The role emphasizes teaching patients and coordinating discharge planning. Licensure requirements include active RN license (or compact), BLS preferred, ACLS acceptable, and BSN preferred.

Qualifications

• Current RN license in state they are working or covered by compact
• BLS Basic Life Support preferred upon hire; ACLS acceptable in lieu of BLS
• Medical-Surgical Nursing Certification RN-BC and other advanced certifications preferred
• Bachelor of Science in Nursing preferred
• One year RN experience required; new grads accepted; 6 months acute care preferred

Responsibilities

• Assess and document complete patient assessments and care plans.
• Administer medications safely per policy and state regulations.
• Coordinate discharge planning with interdisciplinary team.
• Advocate for patient safety and participate in quality improvement.
• Educate patients and families about care plans and post-discharge needs
