Job summary

Brookside Healthcare Center in Cincinnati, OH is seeking an RN MDS Nurse / RAC to join our team on a full-time day shift. The MDS Nurse RAC will complete MDS assessments and coordinate the RAI process, ensuring Medicare/Medicaid oversight and compliant care within CommuniCare guidelines.

The ideal candidate will be a Graduate of an accredited nursing program with RN or LPN license in the state, plus 3 years’ long-term care experience and MDS/RAI/case management familiarity.

Qualifications

• Graduate of an accredited school of nursing.
• RN or LPN
• Valid RN or LPN license in the state employed.
• Three years of experience in a long term care environment preferred.
• Experience with the MDS/RAI process and/or case management preferred.

Responsibilities

• The MDS Nurse RAC reports to the Executive Director and is responsible for accurate and timely completion of MDS assessments and coordination of the RAI process.
• Provides Medicare, Medicaid (case mix), and managed care oversight to ensure appropriate clinical services are provided and appropriate reimbursement is received for each resident.
• Ensures centers are in compliance with federal and state regulations and CommuniCare guidelines and policies; serves as a key member of the facility's management team.

This includes ensuring that the centers are in compliance with federal and state regulations as well as the CommuniCare Family of Companies’ guidelines and policies and procedures. This role serves as a key member of the facility’s management team in helping the facility obtain/maintain quality outcomes.
