Job summary

Proforma Health is seeking a skilled MDS Nurse to accurately complete MDS/RAI components and related documentation for residents. You will ensure assessments mirror clinical status to support individualized care plans and compliance with federal and state regulations.

Ideal candidates have RN licensure, CPR certification, and experience with PDPM, ICD-10 coding, and care planning processes. Strong independent functioning and leadership are valued traits for this role.

Qualifications

• Prior MDS/RAI experience preferred.
• Knowledge of RAI guidelines, PDPM, ICD-10 coding, Quality Measures, and care planning processes.
• Understanding of RAI process, PPS principles, and Case Mix systems.
• Familiarity with regulatory standards and compliance.
• Strong independent functioning and leadership skills.

Job description

The MDS Nurse is responsible for accurately completing Resident Assessment Instrument (RAI) components, including the Minimum Data Set (MDS), Care Area Assessments (CAAs), and care planning documentation, ensuring compliance with federal and state regulations. They ensure assessments accurately reflect residents' clinical status to support individualized care plans.

Qualifications Include

• Prior MDS/RAI experience preferred
