Job summary

CommuniCare Health Services’ Hanover Healthcare Center seeks an RN MDS Nurse Coordinator to join our operations team. You will oversee MDS assessments, coordinate the RAI process, and ensure compliant, high-quality resident care.

The ideal candidate is an RN with long-term care experience and knowledge of Medicare/Medicaid case management. This full-time role reports to the Executive Director and helps maintain strong outcomes.

Qualifications

• Graduate of an accredited school of nursing; RN
• Valid RN license in the state employed
• Three years of experience in a long term care environment preferred
• Experience with the MDS/RAI process and/or case management preferred

Responsibilities

• The MDS Nurse RAC coordinates timely completion of MDS assessments and RAI process
• Provides Medicare/Medicaid oversight to ensure appropriate clinical services and reimbursement
• Ensures compliance with federal/state regulations and company policies
• Acts as a key member of the facility's management team to achieve quality outcomes
