Job summary

Avir Trilogy in Hearne, TX is seeking an MDS (RN) Nurse to join our interdisciplinary care team. You will conduct resident assessments, develop and evaluate care plans, and coordinate with providers to ensure high-quality life for residents.

Responsibilities

• Conduct initial and comprehensive resident assessments and timely updates to ensure care plans meet needs and preferences.
• Develop, implement, and evaluate individualized care plans with the interdisciplinary care team.
• Coordinate with healthcare providers, families, and other stakeholders to ensure continuity of care.
• Provide hands-on nursing care, including administering medications, treatments, and therapies, and assisting with activities of daily living.
• Monitor and manage resident health status, report changes to charge nurse or physician.
• Participate in quality improvement initiatives and maintain accurate records of resident care and services.
