Job summary

Qualifications

Responsibilities

• Conduct initial and comprehensive resident assessments and updates to ensure care plans meet residents' needs and preferences.
• Develop, implement, and evaluate individualized care plans with the interdisciplinary care team.
• Coordinate with healthcare providers, families, and other stakeholders to ensure continuity of care.
• Provide hands-on nursing care, including administering medications, treatments, and therapies, and assist with daily living activities.
• Monitor and report changes in resident health status to the charge nurse or physician.
• Participate in quality improvement initiatives and maintain accurate records of resident care and services.
