Job summary

Mercer Healthcare Center, a member of the CommuniCare Family of Companies, in Bluefield, WV, is seeking an RN MDS Nurse / RAC to join our team. You will lead accurate MDS assessments and coordinate the RAI process to ensure quality resident care.

The ideal candidate is an RN with PPS/MDS experience, CPR certification, and strong clinical and organizational skills. This full-time role offers competitive wages and a supportive team environment within CommuniCare's nationwide network.

Qualifications

• Graduate of an accredited school of nursing.
• RN license in the state employed.
• Prefer experience with both MDS and PPS.
• CPR certification required, or obtained within 90 days of hire.
• Strong clinical assessment and organizational skills.
• Three years of long-term care experience preferred.
• Experience with the MDS/RAI process and/or case management preferred.

Responsibilities

• Complete MDS assessments timely and coordinate the RAI process.
• Provide Medicare, Medicaid (case mix), and managed care oversight to ensure appropriate services and reimbursement.
• Ensure centers comply with federal/state regulations and CommuniCare guidelines.
• Act as a key member of the facility's management team to achieve quality outcomes.

QUALIFICATIONS & EXPERIENCE REQUIREMENTS

• Graduate of an accredited school of nursing
• RN Valid RN license in the state employed
• Prefer experience with both MDS and PPS.
• Must possess and maintain current CPR certification, or acquire a CPR certification within 90 days of date of hire.
• Must possess excellent clinical assessment and organizational skills
• Three years of experience in a long term care environment preferred
• Experience with the MDS/RAI process and/or case management preferred

Other Specific Requirements

• Must possess the ability to make independent decisions when circumstances warrant such action
• Must be knowledgeable of reimbursement regulations and nursing practices and procedures, as well as laws, regulations, and guidelines pertaining to long‑term care administration.
• Must possess the ability to deal tactfully with staff, residents, visitors and the general public.
• Must be knowledgeable of computer systems, system applications, and other office equipment.
• Occasional travel will be required and occasional overnight travel may be required.

JOB RESPONSIBILITIES

• The MDS Nurse RAC (Resident Assessment Coordinator) reports to the Executive Director and is responsible for accurate and timely completion of mds assessments and coordination of the RAI process.
• Provides Medicare, Medicaid (case mix), and managed care oversight to ensure appropriate clinical services are provided and appropriate reimbursement is received for each resident.
• This includes ensuring that the centers are in compliance with federal and state regulations as well as the CommuniCare Family of Companies’ guidelines and policies and procedures.
• This role serves as a key member of the facility’s management team in helping the facility obtain/maintain quality outcomes.
