Job summary

CommuniCare Family of Companies is seeking an RN MDS Nurse to join our team at our Strongsville, OH facility. The role is part-time with a full-time MDS Nurse partner, and there is no on-call or floor nursing required.

Responsibilities include accurate MDS assessments and coordinating the RAI process, ensuring compliance with regulations, and supporting the facility’s management team to achieve quality outcomes.

Qualifications

• Graduate of an accredited nursing program.
• Active RN license in the state of employment.
• Experience with MDS/RAI processes and/or case management preferred.

Responsibilities

• MDS Nurse RAC reports to the Executive Director and completes MDS assessments and coordinates the RAI process.
• Provides Medicare/Medicaid oversight to ensure appropriate services and reimbursement.
• Ensures facility compliance with federal/state regulations and company guidelines.

QUALIFICATIONS & EXPERIENCE REQUIREMENTS

• Graduate of an accredited school of nursing;
• RN Valid RN license in the state employed;
• Three years of experience in a long term care environment preferred;
• Experience with the MDS/RAI process and/or case management preferred.

JOB RESPONSIBILITIES

• The MDS Nurse RAC (Resident Assessment Coordinator) reports to the Executive Director and is responsible for accurate and timely completion of mds assessments and coordination of the RAI process.
• Provides Medicare, Medicaid (case mix), and managed care oversight to ensure appropriate clinical services are provided and appropriate reimbursement is received for each resident.
• This includes ensuring that the centers are in compliance with federal and state regulations as well as the CommuniCare Family of Companies’ guidelines and policies and procedures.
• This role serves as a key member of the facility’s management team in helping the facility obtain/maintain quality outcomes.
