Job summary

The Colony Healthcare Center in Tallmadge, OH is recruiting an RN MDS Nurse to join our team. The MDS Nurse reports to the Executive Director and coordinates the RAI process to ensure accurate assessments and appropriate clinical services.

As a key member of our operations team, you will help ensure regulatory compliance and optimal reimbursement for residents while upholding our commitment to high-quality, compassionate care in a post-acute setting.

Qualifications

• Graduate of an accredited school of nursing.
• RN license in the state employed.
• Three years of experience in a long term care environment preferred.
• Experience with the MDS/RAI process and/or case management preferred.

Responsibilities

• The MDS Nurse reports to the Executive Director and is responsible for accurate and timely completion of MDS assessments and coordination of the RAI process.
• Provides Medicare, Medicaid (case mix), and managed care oversight to ensure appropriate clinical services and reimbursement for each resident.
• Ensures centers comply with federal and state regulations and CommuniCare guidelines and policies.
• Serves as a key member of the facility’s management team to help obtain/maintain quality outcomes.
